Note: Our registered, active SFTR mothers (in a program or attends monthly support groups) will receive assistance. If you DO NOT attend support groups or are NOT in the Mentorship Program, please DO NOT complete this request form for 2025 School Supplies. You will be notified when your supplies are available for pick-up. Thank you!
